Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join the team at Living Center North. The ideal candidate will provide quality nursing care to residents in a long-term care setting while ensuring patient safety, comfort, and well-being.

Responsibilities

• Provide direct and indirect skilled nursing care to assigned residents.
• Administer medications and treatments in accordance with established policies and procedures.
• Monitor residents' health conditions and report changes to the Registered Nurse.
• Maintain accurate and timely patient documentation.
• Collaborate with physicians, registered nurses, and other healthcare staff.
• Deliver age-appropriate care to adult and geriatric residents.
• Follow facility policies, nursing standards, and safety protocols.
• Participate in continuing education and professional development activities.

Requirements

• Active New York State LPN license.
• Current BLS certification (AHA or RQI required).
• Minimum 1 year of experience as an LPN.
• Previous long-term care or skilled nursing facility experience preferred.
• Strong communication and patient-care skills.

Additional Details

• Full-time evening shift.
• 5 shifts per week, 8 hours per shift.
• Every other weekend and holiday availability required.
• Contract-to-permanent opportunity available.
